Important Practice Update

Unfortunately, Dr. Rizzo is no longer seeing patients as part of a clinical practice. He sincerely thanks all his patients for the opportunity to have provided them with quality care spanning many years, and values the resulting relationships he has built.

We are aware of issues that have been encountered lately while trying to reach us appropriately.  Please understand that changes have now been made with our communication process.  We apologize for any inconvenience that may have been caused.

REQUESTS FOR CHARTS and MEDICAL RECORDS:

BY ATTORNEY OFFICES AND RECORD REQUEST SERVICES:

Attorney offices need to make all chart requests for Rizzo Orthopedics / Dr. Charles Rizzo via Chart Swap www.ChartSwap.com

If you have previously emailed a chart request directly to our office, we ask that it be resubmitted through Chart Swap.

Please note that medical records are only available for encounters with Dr. Rizzo from December 2020 on.   Records needed for the period of time prior to December 2020 need to be addressed to Shore Orthopaedic Group.

BY PATIENTS FOR PERSONAL USE:

As required by law, and under HIPPA regulations,  patients may obtain a copy of their medical record with the completion of a patient authorization form. Requests by patients for personal medical records can be made emailing info@RizzoOrthopedics.com.  A Patient Release form will be emailed to you. Upon it’s return to us,  appropriate arrangements will be made to fulfill your request.

DEPOSITIONS and LEGAL REPORTS:

As a continued commitment to his past patients,  Dr. Rizzo will of course continue to provide expert medical testimony by the way of depositions and narrative reports.  If you are an attorney’s office and require assistance, please email Jrizzo@RizzoOrthopedics.com to make necessary arrangements
